One of the most common questions we hear at iSmile is some version of, “Am I actually able to get implants, or am I too far gone for that?” It’s a fair question — for a long time, dental implants were reserved for a narrow group of patients. Today, thanks to advances in bone grafting, full-arch techniques, and treatment planning, the vast majority of people missing one tooth or an entire arch have a path to implants. You can also try our online candidacy assessment — but here’s what actually determines candidacy.
The Basics: What Makes Someone a Good Candidate
In general, you’re likely a strong candidate for dental implants if:
- You’re missing one or more teeth and want a permanent, stable replacement
- Your gums are healthy or can be brought to a healthy state with treatment
- You have (or can rebuild) enough jawbone to support an implant
- You’re in reasonably good general health and can tolerate a minor surgical procedure
- You’re committed to good oral hygiene during healing and afterward
Notice that “enough jawbone” says can rebuild — that’s an important distinction, because bone loss is one of the biggest myths standing between patients and implants.
Common Concerns That Don’t Automatically Disqualify You
“I’ve been missing teeth for years and think I’ve lost too much bone.” Bone loss after tooth loss is normal and expected, but it doesn’t rule out implants. Bone grafting procedures can rebuild the density and volume needed to support an implant, and full-arch techniques like All-on-4 are specifically designed to work with less bone by anchoring implants at strategic angles into the strongest parts of the jaw. “I have diabetes, osteoporosis, or another chronic condition.” Many patients with well-managed chronic conditions are excellent implant candidates. What matters most is how well-controlled the condition is — for example, well-managed diabetes generally doesn’t prevent successful implant placement, though it may affect healing time. This is exactly the kind of thing we evaluate case by case during your consultation.
“I’m a smoker.” Smoking can slow healing and increase the risk of implant failure, but it’s not an automatic disqualifier. Many practices, including ours, work with patients on a plan that may include reducing or pausing smoking around the time of surgery to give the implant the best chance to integrate successfully.
“I’m older and worried I’ve missed my window.” There’s no upper age limit for dental implants. What matters is your overall health and ability to heal, not the number of candles on your birthday cake. Some of our most satisfied patients received their implants later in life after years of struggling with loose dentures.
“I’ve worn dentures for a long time and I’m not sure implants would even work for me now.” This is one of the most common situations we see, and it’s very treatable. Long-term denture wearers often experience significant bone loss, but full-arch implant solutions are frequently designed specifically for this group, replacing an entire arch of teeth with a fixed, permanent set anchored to just a few strategically placed implants. What We Actually Evaluate at Your Consultation
Since no two mouths are alike, candidacy isn’t something that can be determined from a blog post — it requires a real look at your specific situation. During a consultation at iSmile, we typically evaluate:
- Jawbone density and volume, using digital imaging to see exactly what we’re working with
- Gum health, and whether any periodontal treatment is needed first
- Your bite and the position of remaining teeth
- Your medical history, including medications and chronic conditions
- Your goals, whether that’s replacing a single tooth or restoring a full smile
From there, we design a treatment plan tailored to you — not a generic protocol. Curious what your total investment might look like once you’re cleared for treatment? See our breakdown of the cost of dental implants.
The Bottom Line
If you’ve been told “no” before, or you’ve simply assumed implants weren’t an option for you, it’s worth getting a second opinion. Dentistry has changed a lot in the last decade, and solutions that didn’t exist a few years ago may now make implants very achievable for your situation.
